Disciplinary Action / Termination: The host organization or school reserves the right to remove the student from placement for cause, including breaches of safety, misconduct, or nonperformance. Removal from placement may affect course credit and will be processed in accordance with school policy.

Acknowledgment & Certification

By signing below, the student (and parent/guardian if student is a minor) certifies that the information provided on this document is true and complete. The student acknowledges that participation in the placement is subject to the terms set forth in this document and to any additional placement-specific written rules issued by the host organization. The student releases the school from liability for activities beyond the school's direct control, except where the school is negligent in supervising placement obligations.

Indemnification: To the extent permitted by law, the student and parent/guardian agree to indemnify and hold harmless the host organization and the school from third-party claims arising from the student's willful misconduct or negligent acts during the placement.

Photo/Media Release: I permit the school and host organization to use photographs or video taken during placement for educational and promotional purposes, without compensation, unless I opt out in writing.
