I, the undersigned, acknowledge that participation in the Activity may involve inherent risks, including but not limited to transportation, physical exertion, contact with other participants, and environmental hazards. I understand and voluntarily accept these risks.

In consideration of being permitted to participate in the Activity, I hereby release, waive, discharge, and covenant not to sue the School / Organization, its officers, employees, volunteers, agents, and representatives (collectively, the Released Parties) from liability for any and all claims, demands, causes of action, or suits for bodily injury, property damage, wrongful death, or other loss arising out of or in any way connected with my participation in the Activity, whether arising from the negligence of the Released Parties or otherwise, to the fullest extent permitted by law.

I further agree to indemnify, defend, and hold harmless the Released Parties from and against any and all losses, liabilities, damages, costs, and expenses (including reasonable attorneys' fees) resulting from any claim or action brought by or on behalf of me or my child arising out of the Activity.

If the undersigned is a minor or if emergency treatment is necessary and I am unavailable, I authorize the Released Parties to obtain medical treatment for the participant. I understand that reasonable attempts will be made to contact the emergency contact or parent/guardian prior to non-emergency treatment when practicable.

Transportation & Supervision

I consent to transportation arrangements organized by the School / Organization and understand that transportation may be by school vehicle, charter vehicle, or private vehicle driven by authorized volunteers or staff. The School / Organization will make reasonable efforts to verify driver qualifications and insurance.

Media Release & Privacy

I grant permission for photographs, video, or audio recordings of the participant to be taken during the Activity and used by the School / Organization for educational, promotional, or archival purposes without compensation. I understand personal contact information will not be published without additional consent.

Code of Conduct & Acknowledgment

I agree that the participant will follow all safety instructions, rules, and directions given by staff and volunteers. Failure to comply may result in removal from the Activity without refund and notification to parent/guardian.
