SUMMER CAMP ENROLLMENT FORM

This Summer Camp Enrollment Form constitutes an enrollment agreement between the enrolling party and the Camp Operator. Submission of this form by the parent or legal guardian and receipt of any required deposit constitutes the registrant's agreement to all terms, fees, policies, and authorizations contained herein.

Authorization: In the event of emergency and if the parent/guardian cannot be reached, I authorize the Camp Operator to obtain emergency medical care for the student and to consent to any medical treatment deemed necessary by emergency medical personnel. I agree to be responsible for all costs incurred for such treatment.

Tuition, Cancellation & Refund Policy

A non-refundable deposit is required to reserve space in any session. If a registered student withdraws at least 30 days prior to the start of a session, tuition paid beyond the deposit will be refunded less any administrative fees; withdrawals made within 30 days of session start are not entitled to refund except at the sole discretion of the Camp Operator and subject to an administrative fee. No refunds are issued for days missed due to illness, disciplinary suspension, or family vacation. The Camp Operator reserves the right to cancel sessions due to insufficient enrollment or circumstances beyond its control; in such a case a full refund or credit toward another session will be issued.

Late pickup fees will be charged for children not collected by scheduled pickup time. Returned check or rejected electronic payment fees will be charged and may affect enrollment standing.

Code of Conduct & Discipline

The student agrees to adhere to the Camp's behavioral standards. The Camp Operator may suspend or dismiss any participant whose conduct is detrimental to the program or others. Dismissal for disciplinary reasons does not entitle the family to a refund. Repeated or serious misconduct may result in permanent exclusion from future programs.

Photo / Media Release

I authorize the Camp Operator to photograph and/or record the student during camp activities and grant the Camp Operator a non-exclusive, royalty-free license to use such images or recordings for program promotional and educational purposes. If you do NOT agree, check the box below.

Transportation Authorization

The undersigned authorizes the Camp Operator to transport the student in camp vehicles for scheduled activities and to transport the student to emergency medical facilities if necessary.   Authorized

Liability & Indemnification

To the fullest extent permitted by law, the registrant releases, holds harmless, and indemnifies the Camp Operator, its employees, volunteers, and agents from all claims, liabilities, losses, costs, and expenses arising out of participation in camp activities, except to the extent caused by the gross negligence or willful misconduct of the Camp Operator. This release includes, but is not limited to, claims for personal injury and property damage.
