Attendance, Behavior, and Dismissal: I acknowledge that regular attendance is required. The program maintains a code of conduct; serious or repeated misconduct may result in dismissal without refund. The program reserves the right to dismiss a participant for health or behavior reasons to protect others.

Cancellation & Refunds: Enrollment cancellations made in writing before the published deadline are eligible for a refund less administrative fees. Cancellations after the deadline, or dismissal for cause, may not be eligible for refund. I accept responsibility for any unpaid balances in accordance with the payment plan selected.

Photo and Media Release: I grant permission for the program to photograph or record the participant for promotional, educational, or archival purposes. I understand no compensation will be provided. If I do not consent, I will indicate by checking the box below.

Liability Release and Indemnification: In consideration of participation, I release the program, its employees, volunteers, and agents from claims arising from ordinary negligence. I agree to indemnify and hold harmless the program from claims, demands, or causes of action arising out of the participant's actions, except to the extent caused by gross negligence or willful misconduct by the program. This release applies to travel to and from program activities and on-site activities.

Emergency Treatment Authorization: If I cannot be reached in an emergency, I authorize program staff to secure necessary medical treatment for the participant, including hospitalization and anesthesia, as may be required by the treating physician, and I agree to be financially responsible for such treatment.
