TRACK COACHING AGREEMENT

Coaching Services

Coach shall provide individualized and/or group track coaching services including technique instruction, workout planning, conditioning, and meet preparation (Services) at the locations and on the schedule set forth below. Services will be provided in a manner consistent with accepted coaching standards and the Coach's professional judgment.

Fees, Payment & Refunds

Athlete shall pay Coach the fees set forth below in consideration for Services. Fees are earned on delivery of Services. Any payment plan is a separate repayment obligation; late payments may incur a late fee and/or suspension of Services until brought current.

Refunds: Except as otherwise required by law, refunds will be issued only where Coach cancels Services and shall be pro rata for sessions not yet provided. Cancellations by Athlete require at least days' written notice to be eligible for any prorated credit.

Attendance, Make-up & Conduct

Athlete is expected to attend scheduled sessions. If Athlete misses a session, Coach will offer a make-up only at Coach's discretion and subject to availability. Repeated absence, disruptive conduct, or failure to follow reasonable coaching instructions may result in suspension or termination of Services without refund.

Medical Information & Emergency Authorization

By signing below, Parent/Guardian or Athlete authorizes Coach and Coach's representatives to consent to emergency medical treatment if reasonable attempts to contact Parent/Guardian cannot be made. Such authorization does not create liability for Coach for acts performed in good faith.

Media Release

Athlete and Parent/Guardian grant permission for Coach to use photographs, video, and performance information for coaching, promotion, and instructional purposes. Check one:

Confidentiality & Use of Data

Coach may collect training data and performance metrics. Coach will treat Athlete's personal and health information as confidential and will not disclose such information except as required to provide Services, to comply with law, or with Athlete's prior written consent.

Equipment, Uniforms & Travel

Athlete is responsible for bringing appropriate footwear, apparel, and any personal equipment. For travel to meets or off-site sessions, Parent/Guardian must provide consent for travel and any associated release of liability when requested in writing.

Cancellation, Termination & Amendment

This Agreement may be terminated by either party for material breach if the breach is not cured within 14 days after written notice. Coach may terminate immediately for safety concerns or misconduct. Amendments to this Agreement must be in writing and signed by both parties.

Liability, Release & Indemnity

Athlete and Parent/Guardian acknowledge that participation in track training involves inherent risk of injury. To the fullest extent permitted by law, Athlete and Parent/Guardian release, waive and discharge Coach and Coach's agents from liability for injury, death or damage arising out of or connected with the Services, except for acts of gross negligence or willful misconduct. Athlete and Parent/Guardian agree to indemnify and hold harmless Coach from any third-party claims arising from Athlete's conduct.

Governing Law & Dispute Resolution

This Agreement is governed by the laws of the jurisdiction in which Coach operates. The parties agree to attempt resolution of disputes in good faith through mediation prior to initiating litigation. Remedies available to either party shall be in addition to any other legal remedies.
