This Training Enrollment Order authorizes enrollment in the listed course(s) and constitutes a binding agreement between the Trainee (or Parent/Guardian if Trainee is a minor) and the Training Provider or Authorizing Organization. Enrollment is conditional upon receipt of required payment or valid authorization reference. The Trainee certifies that all information provided in this form is true and complete and authorizes release of records as required for enrollment administration and certification issuance.

Attendance, Completion & Certification: The Trainee must meet attendance and assessment requirements established by the Provider to receive any certificate or credential. The Provider may withhold certificate issuance for noncompliance with attendance, academic integrity, or financial obligations.

Cancellation & Refunds: Cancellation by the Trainee prior to the first scheduled session will be subject to the Provider’s published cancellation schedule and any non-refundable administrative fees. No refunds for no-shows. Employer or Authorizing Organization remains responsible for payment if an authorization or PO was provided and not revoked in writing prior to the Provider’s stated deadline.

Liability, Indemnity & Equipment: The Provider is not liable for personal injury beyond negligence standards of the jurisdiction. Trainee agrees to return borrowed equipment in good condition and to indemnify the Provider for loss or damage caused by willful misconduct.

Data & Privacy: Trainee records related to enrollment and completion will be maintained for administrative and regulatory purposes. By signing, Trainee consents to processing of personal data necessary to administer the program and issue certifications.

Governing Law & Dispute Resolution: This Order shall be governed by applicable law where the Provider operates. Disputes arising under this Order shall be resolved in the Provider’s customary administrative procedures; absent resolution, disputes may be submitted to a court of competent jurisdiction.
