Enrollment Acceptance: By signing this Training Enrollment Statement, the registrant (or parent/guardian) accepts placement in the named program and agrees to comply with the program schedule and instructional requirements. The training provider reserves the right to modify schedule times or instructors when necessary; registrant will be notified in writing of significant changes.

Tuition, Fees and Payment: Tuition and associated fees are due as described above. Failure to make timely payment may result in suspension from classes and withholding of completion documentation. Late payments may incur administrative fees and interest as set by the provider. The registrant authorizes collection remedies for unpaid balances.

Cancellation and Refunds: If the registrant withdraws prior to the course start date, refunds of tuition less a non-refundable administrative deposit will be issued as provided in the provider's refund policy. After course commencement, refunds are calculated pro rata subject to instructional services rendered and any non-recoverable costs. Requests for withdrawal must be submitted in writing.

Attendance and Completion: Satisfactory completion requires meeting attendance and competency standards. Missed instruction may require make-up work or additional fees. Completion certificates are issued only upon fulfillment of curriculum and financial obligations.

Liability and Indemnity: The registrant (or parent/guardian) releases and holds harmless the provider, its officers and employees from claims arising from ordinary risks of participation in training activities, except to the extent caused by gross negligence or willful misconduct. The registrant agrees to indemnify the provider for third-party claims arising from registrant's misconduct.

Emergency Medical Authorization: In the event of an accident or medical emergency, if the provider cannot contact the emergency contact, I authorize the provider to obtain medical care for the registrant and agree to be financially responsible for such care.
