Scheduling Confirmation: By submitting this form the participant requests the training session(s) described above. The institution will confirm a scheduled date in writing. The confirmed date and time constitute the binding schedule subject to the cancellation and rescheduling terms below.

Cancellation and Rescheduling: Participant acknowledges that cancellations made fewer than 10 calendar days before a confirmed session may incur a cancellation fee equal to 50% of the estimated total fee. Rescheduling requests made fewer than 5 calendar days before the confirmed session are subject to availability and may incur additional administrative charges.

No-Show and Late Arrival: Participant understands that late arrival or failure to attend may result in forfeiture of the session without refund. The institution is not responsible for missed content in shortened or abbreviated sessions due to late arrival.

Liability and Indemnification: To the fullest extent permitted by law, participant releases and agrees to indemnify the institution, its officers, employees, and agents from and against any claims, damages, losses, or expenses (including reasonable attorneys' fees) arising from participation in the training, except to the extent caused by the institution's gross negligence or willful misconduct.
