CONSENT, ASSUMPTION OF RISK & RELEASE

I, the undersigned, on behalf of myself, my heirs, executors and assigns, acknowledge that participation in interscholastic athletics involves risk of physical injury which may be severe and may result in permanent disability or death. I accept and assume all such risks, both known and unknown, and assume full responsibility for medical or related expenses arising from participation.

I give permission for emergency medical treatment to be administered to the student if necessary. I authorize school or team officials to seek medical care, transport the student to appropriate medical facilities, and to provide medical information to treating providers as necessary. I release the school district, its employees, coaches, volunteers and agents from liability for ordinary negligence in the provision of such care, to the extent permitted by law.

I agree to comply with all team rules, school policies, and athletic association regulations including but not limited to rules regarding conduct, substance use, and drug testing protocols when applicable. I understand that violation of these policies may result in disciplinary action, including suspension or dismissal from the team.

MEDIA RELEASE & PRIVACY

From time to time the student may be photographed, filmed or interviewed during athletic activities. Please indicate consent for the use of the student’s image or voice in school-published materials or media produced by the school.

FEES, PAYMENT & REFUND POLICY
