In consideration of enrollment in welding instruction, Student Name: hereby acknowledges the known and unknown risks associated with welding and related activities. Student voluntarily assumes all risks of injury, damage, or loss arising from participation in the training.

Student releases, waives, and discharges the training provider, its officers, employees, agents, and instructors from any and all claims, demands, actions, or causes of action for injury, death, or property damage arising out of participation, except to the extent caused by gross negligence or willful misconduct of the training provider. Student agrees to indemnify and hold harmless the training provider against any third-party claims resulting from Student's negligent acts.
