I authorize Yin Foundations and its staff to obtain emergency medical treatment for the student if, in the judgment of those present, such treatment is necessary. I agree to be responsible for any costs incurred in the event of emergency treatment.

Policies, Waiver and Acknowledgements

Attendance and conduct: Students are expected to arrive prepared and on time for scheduled sessions. Chronic tardiness or disruptive conduct may result in suspension or dismissal from the program without refund. The student agrees to follow instructor directions for safe practice and to notify instructors of any discomfort or injury.

Cancellation and refund: Enrollment confirmed upon receipt of deposit. If the student withdraws more than 14 days prior to the program start date, the deposit will be refunded less a processing fee. Withdrawals within 14 days of start date and after the program begins are subject to the refund schedule provided in the program materials. No refunds are provided for missed classes once the program has commenced, except at the sole discretion of Yin Foundations.

Release of liability: To the fullest extent permitted by law, the student (or parent/guardian on behalf of the student) releases and holds harmless Yin Foundations, its instructors, employees and agents from any and all liability, claims, demands, or causes of action arising out of injury or illness sustained as a result of participation in the program, except where such injury arises from gross negligence or willful misconduct of Yin Foundations.

Photo and media release: I grant Yin Foundations permission to photograph or record the student during program activities and to use such media for educational, promotional, or archival purposes unless I indicate otherwise below.
