Medical Treatment Consent: I authorize retreat staff and authorized medical personnel to administer or secure emergency medical treatment for the participant if necessary. I understand reasonable efforts will be made to contact the emergency contact prior to treatment when practicable. I agree to be financially responsible for any costs incurred.

Assumption of Risk and Release: Participation in retreat activities involves inherent risks. In consideration of acceptance of enrollment, I, on behalf of participant and our heirs, release and hold harmless the Retreat Provider, its employees, volunteers, and agents from any and all liability, claims, demands, causes of action, whether arising from negligence, breach of contract, or otherwise, that I or the participant may have for bodily injury, property damage, or wrongful death arising out of participation, except for willful misconduct or gross negligence.

Photo/Video Release: I grant permission for photographs, video, or other media of the participant made during the retreat to be used by the Retreat Provider for promotional, instructional, or archival purposes. No compensation will be provided for such use.

Behavior & Discipline: Participant is expected to comply with all rules, directions of staff, and safety protocols. The Retreat Provider reserves the right to dismiss at the participant's or guardian's expense any participant whose behavior threatens safety or substantially interferes with the program. Dismissal does not entitle participant to a full refund.

Cancellation & Refund Policy: Fees are non-transferable unless expressly agreed in writing. Refunds, if any, are subject to administrative fee deductions and will be determined in accordance with the Provider's stated refund schedule. Special circumstances may be considered on a case-by-case basis.

Governing Law: This Contract will be governed by and construed in accordance with the substantive laws of the jurisdiction in which the Retreat Provider maintains its primary place of business, without regard to conflict of law principles.

Severability: If any provision of this Contract is held invalid or unenforceable, the remainder of the Contract will remain in full force and effect and will be construed so as to effectuate the original intent of the parties to the maximum extent permitted by law.

Entire Agreement: This Contract, together with any written schedules, constitutes the entire agreement between the parties with respect to the subject matter and supersedes all prior and contemporaneous agreements and understandings, whether written or oral.
