DURABLE POWER OF ATTORNEY(To Execute Oil and Gas Lease) Principal: (Name and Address) Agent: (Name and Address) Effective Date: Termination Date: (Indicate "None" if there is no Termination Date) Principal, named above, makes, constitutes, and appoints Agent, named above, as Principal's Agent and Attorney in Fact, granting to Agent the following powers and authority: 1. To execute and deliver, from time to time, an Oil and Gas Lease or Le ases on terms acceptable to Agent covering the following lands (the "Lands"): (Description of Lands to be covered by Lease, including county and state); 2. To accept payments of bonus for executing an Oil and Gas Lease or Leases and all payments including royalties, provide for in any lease; and, 3. To do all acts and sign every instrument necessary or appropriate to lease the Lands for the exploration, production and development of oil and/or gas. Principal ratifies all acts done pursuant to this appointment. Agent's authority shall begin on the Effective Date stated above, and end on the Termination Date stated above, unless revoked sooner by Principal's written statement recorded in the county where the Lands are located. THIS POWER OF ATTORNEY SHALL NOT TERMINATE ON THE DISABILITY OR INCAPACITY OF THE PRINCIPAL PRIOR TO THE TERMINATION DATE.
