The Court orders the following pursuant to §15-14.5-302(6), C.R.S.: 1. The Guardian and/or Conservator shall notify the Court within 30 days if his/her home address, email address, or phone number changes and/or any change of address for the Ward/Protected Person. 2.  The Guardian shall file an Annual Guardian’s Report (JDF 850) on or before (date) beginning in (year) for the duration of the guardianship. 3.  The Conservator shall file an Annual Conservator’s Report (JDF 885) on or before (date) beginning in (year) for the duration of the conservatorship. Copies of all future filings with the Court shall be provided to the following identified as interested persons in this matter, by the one filing such documents. In addition, the Guardian and/or Conservator shall provide a copy of the required reports, to the following interested persons within ten days of filing with the Court.
