For Emancipation Cases: The person who is completing the Voluntary Appearance Form will check the box that indicates if they are the parent, legal guardian, relative or legal custodian of the petitioner. FINAL SIGNATURE (To be completed by t he party completing the Voluntary Appearance) • On the first line, the person completing the Voluntary Appearance should sign his or her first, middle, a nd last names and enter t he date. • On the second line, the party completing the Voluntary Appearance shoul d PRINT his or her first, middle, and last names. • On the third line, th e person completing the Voluntary Appearance should enter his or her full street address. • On the fourth line, the person completing the Voluntary Appearance should enter his or her city, state, and ZIP code. • On the fifth line, th e person completing the Voluntary Appearance should enter his or her telephone number, including the area code, and his or her e-ma il addr ess, if any.
