RULE TO CHANGE CUSTODY FROM SOLE CUSTODY
TO JOINT CUSTODY AND TO AMEND VISITATION
NOW INTO COURT, through undersigned counsel, comes ____________ , who suggests to
the Court the following:
I.
Petitioner and Defendant were divorced by Judgment of this Cour t on the ___ day
of ______ , 20 ___ .

Pursuant to said Judgment, ____________ was granted the sole care, custody and
control of the minor children born of the marriage, namely ____________ and
_______ _____ , subject to ____________ 's specific visitation privileges set forth
therein.
III.
Petitioner, ____________ , now desires the joint custody of said minor children,
with ____________ being designated as custodial parent, and requests that his
visitation privileges set forth in said Judgment be amended as follows:
l. On the 2nd and 4th weekends of each calendar month from
Friday at 4:00 P.M. through and including Sunday at 4:00
P.M.;
2. Alternating major holidays, consisting of Easter, July 4th,
Labor Day, Thanksgiving, Christmas Eve, Christmas Day, and
New Year's Day;
3. The children shall be with their father on Father's Day,
beginning at 9:00 A.M., and ending at 5:00 P.M.;
4. The ch ildren's birthdays shall be shared equally between
the parties;
5. ____________ will have visitation during the summer,
beginning two weeks after school ends and ending two weeks
before school begins, with visitation privileges to
____________ during this time;
6. ____________ will have visitation for three days during the
school h olidays that are in excess of one week in duration,
which includes Easter, Tha nksgiving, Christmas/New Year's;
and
7. During the summer visitat ion period, the child support
payments will be suspended.
WHEREFORE, Petitioner, ____________ , prays that this Honorable Court
order the defendant, ____________ , to show cause on a date and at a time to be fixed
by this Honorable Court, why petitioner and defendant should not be granted the joint
