This form is a guide to help you in meeting various requirements, such as voting, identification card, driver’s license and vehicle registration requirements. VOTING REGISTRATION REQUIREMENTS - In order to register to vote in the state of Indiana, you must meet the following requirements:  You must be a citizen of the U.S.  You must be at least 18 years old on the day of the next general or municipal election.  You must not currently be imprisoned after being convicted of a crime.  You must have lived in your precinct for at least 30 days before the next election.  You must complete an application to register to vote and either mail or hand-deliver it to your county registration office, having it postmarked at least 29 days prior to a primary or general election to vote in that election.  You may apply to vote at your county registration office, circuit clerk office, any license branch, public assistance office or other sites or by mail on a pre-addressed card you can obtain at public libraries, license branches, local school corporations with public secondary schools or local government offices.  If you are voting for the first time by mail, you must include a photocopy of a current valid photo ID, current utility bill, bank statement, government check, paycheck or other document that includes your name and address. IDENTIFICATION CARD REQUIREMENTS - You may obtain an identification card from the Indiana Bureau of Motor Vehicles if you are an Indiana resident who does not drive. The identification card looks similar to a standard driver’s license except that it is clearly marked as a non- driver identification card at the top of the document. You may be any age to obtain a non-driver identification card, but will receive a reduced fee if you are a senior or a disabled applicant. Proof of residency documents include the following:  Any primary or secondary document listed above as long as such document contains your name and residential address  Child support check stub from Indiana Family and Social Services Administration with your name and address  Bill or Benefit Statement within 60 days of issuance  Indiana driver’s license, ID card or permit with photo  Indiana surveyor report  Indiana residence affidavit  Voter registration card  Valid Indiana vehicle or watercraft title or registration DRIVER’S LICENSE REQUIREMENTS - You must obtain an Indiana driver’s license within 60 days of establishing Indiana residency. In order to obtain a driver’s license in Indiana, you must meet the following requirements:  You must visit an Indiana license branch to obtain a license.  You must surrender your previous license.  If you hold a valid photo license issued by another state, the District of Columbia or U.S. Territory, you must successfully pass a written and vision test. If your out-of-state license is expired over one year, you must also take a drive test. In order to register and title your vehicle in Indiana, you must meet the following requirements:  You must provide the following documents at the license branch to obtain a registration: o Certificate of Origin or Out-of-State Title o Odometer Statement o Proof of Insurance o Verification of Social Security Number o Proof that the vehicle you are registering passed a vehicle emissions test, if you live in Clark, Floyd, Lake and Porter counties.  In addition, you must apply for an Indiana title before the license branch can issue you a license plate.  You will need the following documents to title your vehicle in Indiana: o New vehicle-  Title application  Properly assigned certificate of origin  Completed conforming odometer statement (generally found on the certificate of origin)  Proof of sales tax paid (i.e. ST108 form from dealer) and  VIN Inspection (if vehicle is from out-of-state dealer). o Used vehicle-  Properly assigned certificate of title  Completed conforming odometer statement  Bill of sale and  VIN Inspection (if vehicle is from out-of-state dealer).
