Petitioner desires to remove the body of Decedent from the mausoleum in the above-named cemetery and to transfer and reinter the body in Petitioner's family plot also located in Respondent's cemetery, which plot of land was also owned by Decedent, and is located near the plot of land on which the mausoleum is erected, and which plot is designated as Plot No. _______, Section ______, containing _________________ ( number of square feet) , which is sufficient space for ____________ (number) graves . 6. On _________________________________ (date) , Petitioner requested Respondent to allow the removal of Decedent's body in the manner alleged in Paragraph 5, and presented Respondent with the signed consent to the removal and reinterment made by Petitioner and the children of Decedent, who are all above the age of 21, but Respondent refused and still refuses to allow Petitioner to remove and reinter Decedent's body.
