Contract or Agreement for Piano Lessons This contract is hereby made this ____day of ___________________, 20___ between ______________________________________ (Client) and _________________________ ________________ (Parent of Client) , who reside at _______________________________ ____________________________________________________________ (street address, city, county, state, zip), both of whom are hereinafter jointly and severally referred to as Client, and ______________________________________________ (Teacher) , who resides at _________ _____________________________________________________________ (street address, city, county, state, zip) , hereinafter referred to as Teacher . 1. Client hereby hires Teacher for ________________ (e.g., 5 one-hour) piano lessons at $_____________ per hour for the ______________________ (e.g., Spring 20___) Academic Semester. The date(s) and time(s) for the aforementioned lessons are listed below. Dates Times (Day of the week), the ___ day of _________________, 20___, _______p.m. - _______p.m. (Day of the week), the ___ day of _________________, 20___, _______p.m. - _______p.m. (Day of the week), the ___ day of _________________, 20___, _______p.m. - _______p.m. (Day of the week), the ___ day of _________________, 20___, _______p.m. - _______p.m. At least one week = s advance notice will be given to Client before any changes are made. 2. If Client must cancel a lesson, 24 hour notice must be given to Teacher or the fee of $______________ will be immediately due and payable. A good faith effort will be made by Client and Teacher to reschedule a make-up appointment for the same week. Make-up sessions cannot be scheduled for dates beyond the current week. Should Teacher need to cancel a session, Client may reschedule for the same week. 3. If Client is not present at the pre-determined location when Teacher arrives or is more than fifteen (15) minutes late, he will still responsible for payment for the lesson. Until full payment is made, all future appointments will be suspended. Clients who arrive late will only be given a lesson for the remaining hour.
