Providers may begin using the revised form immediately. The HCY Lead Risk Assessment Guide and its instructions may be found on the Division of Medical Services’ Web site at http://dss.missouri.gov/dms/providers.htm by clicking on the 'Medicaid Forms" option in the left column. The form may also be found on the Department of Health & Senior Services’ Web site at http://www.dhss.mo.gov/ChildhoodLead/. The guide has been revised to include the following changes: • • • • • • • • The form lists the Department of Health and Senior Services and the Department of Social Services as partners in the screening guide. The form will be able to be used by both a Medicaid and a non-Medicaid patient as a new section allows the medical insurance information for non-Medicaid patients. Name of the person completing the verbal questionnaire. Check boxes for whether or not the tested child lives in a high-risk area. Space for the parent’s signature if they choose not to have the questionnaire completed or the child tested. Spaces for recording the physician’s name and phone number. Spaces for recording the parent/guardian’s name, address, and phone number.
