INSTRUCTION FOR COMPLETING FWS FORM­3­2354 QUOTA DEER HUNT APPLICATION Please follow the instructions below when submitting an application for the Pinckney Island NWR Gun Hunt. Only one application for each hunt may be submitted per hunter. Persons or groups submitting more than one application per person will be disqualified. Hunters may apply in parties of up to three individuals. Each hunter must fill out his/her own application, and then all applications must be stapled and mailed in together. All applications must be received by August 31, 2013 at the following address: Hunt Program Savannah Coastal Refuges Complex 694 Beech Hill Lane Hardeeville, SC 29927 This is a fillable form, meaning you can enter your information using a computer if that computer has Adobe Reader software (most computers now come with this installed or it can be downloaded for free at www.adobe.com). Once the form is complete, simply print it out (you will not be able to save your information). If you prefer, you may print the blank form and write the information in by hand. Many of the fields on the application are already filled in. If they are, please do not change the information that is already there. The only fields you need to fill in are the following: 2. Date of application: Write or type today’s date 3. Contact Information: Write or type your full, legal name, mailing address, a phone number where you can usually be reached during the day, and your email address, if you have one. 8. Acknowledgement and signature: Once you have completed all fields and printed the form, please sign here. Legal guardian’s signature is required as well for hunters under the age of 18.
