ATTACHMENT TO JUDGMENT FOR CHILD SUPPORT (Note any changes initialed by Judge) I. In accordance with Uniform Chancery Court Rule 8.06 for so long as any child herein is a minor, the parents shall (a) keep each other informed of their respective addresses, including street, house or apartment number, city and state, and telephone number and (b) within five (5) days of any change notify the Clerk of this Court in writing, providing the file number and full address and telephone number and furnishing the other party a copy of such notice. The willful failure of either party to comply with this provision may be treated as contempt of court. II. The rights of the parent who has permanent physical and legal care, custody and control of the minor child(ren) are subject to the following rights of visitation hereby vested in the other parent (the non-custodial parent), including specific modifications by deletion or interlineation hereon, if any, by the court: A. Alternating weekends, beginning Friday, the day of , 20 , commencing at o'clock .m. on and ending at o'clock .m. on the immediate following .B. For two periods of two weeks each during the Summer every year, provided that this shall not interfere with school attendance and provided that written notice shall be given by the non-custodial parent to the custodial parent specifying the particular two periods selected on or before the preceding day of each year.C. During Thanksgiving in even-numbered years, beginning Wednesday before Thanksgiving at o'clock .m. and ending the immediate following at o'clock .m.; during each Christmas holiday season, in even- numbered years beginning at 3333 o'clock .m., on December 25 and ending at o'clock .m. on the following , and in odd-numbered years beginning at o'clock .m. on December 23 and ending at o'clock .m. on ; during Easter in even-numbered years beginning at o'clock .m. on Good Friday and ending at o'clock .m. on Easter Sunday; during the school Spring break in odd-numbered years, from o'clock p.m. on the day school is recessed through o'clock p.m. on the day prior to school resuming.D. Notwithstanding any of the provisions in the above schedule, the mother shall have visitation (or if she is custodial parent she may suspend the non-custodial parent's visitation) from o'clock .m. to o'clock .m. every Mother's Day, and in even- numbered years on her birthday and the child(ren)'s birthday(s) from o'clock .m. to o'clock .m., and the father shall have visitation (or if he is custodial parent he may suspend the non-custodial parent's visitation) from o'clock a.m. to o'clock p.m. every Father's Day, and in odd-numbered years on his birthday and the child(ren)'s birthday(s) from o'clock a.m. until o'clock p.m.E. Visitation may occur at any reasonable time agreed upon by both parents with reasonable advance notice to the custodial parent and child(ren). F. The non-custodial parent shall provide transportation for each minor child from and to the home of the custodial parent for each visitation. III. Each parent shall exert every reasonable effort to maintain free access and unhampered contact between the other parent and each minor child and to foster a feeling of affection between each minor child and the other parent. The non-custodial parent shall not take or allow any action to undermine or interfere with any legal decision by the custodial parent regarding any minor child. Neither parent shall do anything which may estrange any minor child from the other parent, injure the child's opinion of the other parent, or hamper the free and natural development of the child's love and respect for the other parent. IV. There will be cases from time to time when it will be necessary to slightly vary from the terms of the above recommended visitation, in order to comply with the convenience and wishes of the parties. Assuming such slight variances are agreed, the Court will not usually disapprove same. However, if such proposed visitation will constitute a major departure from the recommended visitation schedule outlined above, attorneys may desire to discuss such variations with the Court prior to presentation of the Final Judgment, in order to expedite finalization.
