AFFIDAVIT STATE OF LOUISIANA PARISH OF ____ ____ ____ BEFORE ME, the undersigned authority, personally came and appeared ________________ , who, after being duly sworn, did depose and state the following: (l) That his date of birth is _________ ____ , ____ . That he is a resident of the Parish of _________ , State of Louisiana. (2) That on or about the ____ day of ____ ____ 20 ____ , he was a passenger of a ________________ , bearing VIN # ____ ____ ____ ____ in the Parish of _________ , State of Louisiana. (3) That on or about said date, the vehicle was stopped by the _________ Police Department, Officers _________ and _________ . (4) That the driver of the vehicle, ____ ____ ____ , was asked to remove himself from the vehicle, and the Defendant herein, ________________ , was asked to provide his driver = s license. (5) That he was placed inside of the patrol car for the Officers = safety, and remained there for approximately forty minutes, during which Officers _________ and _________ searched the vehicle and both him and the driver, not recovering anything from either the vehicle or from the Defendants. (6) That Officers _________ and _________ transported Defendants to the _________ Police Department, wherein the Officers informed them that they believed the driver of the vehicle, ________________ , dropped an A irregular shaped @ object, believed to be cocaine. (7) That he and the driver, ________________ , were again searched; however, nothing was recovered by the Officers. (8) Both he and the driver, ________________ , were asked to place test swabs in their mouths, at which time they were asked to leave the room. When they returned, the Officers alleged that the test was positive for cocaine. (9) That he and the driver were then charged with Possession of Cocaine and placed in the custody of the _________ Police Department.
