It is necessary and proper that an Administrator/Administratrix be appointed to

Administrator/Administratrix and is capable and desires to administer the assets of decedent
under the direction of this Court, and no other person has made any effort to qualify as
Administrator/Administratrix of the estate of the decedent.

The estate of the decedent consists of an automobile, real property and retirement.
Petitioners specifically waive a formal inventory, appraisal and bond and request that be
appointed as Administrator/Administratrix of the Estate of , deceased.
