Fill and Sign the Beeper Pager Usage Policy Form

BEEPER/PAGER USAGE POLICY Family pages are to be kept to a minimum. Beeper/pagers generally are to be used only for company business. Employees may not use beeper/pagers to conduct business for others or themselves. Beeper/pagers are not to be used for any illegal activities. All beeper/pager requests must be submitted to your supervisor and must be included in your department's budget. The beeper/pager, the plan, and any accessories must be included on the request along with the business justification for the beeper/pager, the plan, and accessories. Generally, the least expensive beeper/pager and calling plan are to be selected. Usually, contracts for beeper/pager service will be in the company's name. Upon request of either the employee or the company, the beeper/pager may be in the employee's name. In that case, the company will only reimburse the employee for charges related to pages for company business. In all other cases, beepers/pagers and accessories remain company property. To the extent permitted by applicable law, employees will be held responsible for lost or stolen beepers/pagers, etc., belonging to the company. Employees must immediately report lost or stolen beepers/pagers, etc. Generally, confidential information is not to be transmitted over alpha/numeric beepers/pagers. Phones are not to be forwarded to beepers/pagers. Phones may be forwarded only to voice mail or a combination voice mail and beeper/pager. All company policies that apply to regular telephones and other company property apply to beepers/pagers (e.g., no harassing pages). Employees may be disciplined for violating the beeper/pager policy.
