declared an Instrument in writing purporting to be his/her Last Will and Testament. The original

The Decedent made, published, and declared and executed said instrument being of
sound and disposing mind and memory and of legal age to devise. Said instrument is in
conformity with the requirements of law and said instrument constitutes the true and lawful Last
Will and Testament of and it is entitled to probate. The Affidavit of , one
subscribing witness, to the Last Will and Testament of is attached as Exhibit " ".

Petitioner would show that said instrument of writing names as Executor(Trix) of
said Last Will and Testament, and directs that said Executor(Trix) serve without bond and
accounting.

and do hereby request that be appointed as Executor(Trix) to serve without bond,
inventory, appraisal or accounting.

received and the instrument of writing presented herewith be admitted to probate and be filed
and recorded as the Last Will and Testament of , deceased; and
Complaints further pray that Letters Testamentary be issued to , upon his/her
