Contract or Agreement of Employment with Domestic Service Worker Who Lives and Works on the Premises This agreement is made on the ___ day of ____________, 20___ between __________________of ______________________________ (street address, city and state) , referred to as Employer, and ________________________ of __________________________ (street address, city and state), referred to as Employee.
