EDUCATIONAL ASSISTANCE PROGRAM FOR EMPLOYEES WHEREAS, ____________________________ (Employer ) has established an educational program to help eligible employees develop their skills and upgrade their performance; and WHEREAS, educational assistance is provided for courses offered by approved institutions of learning, such as accredited colleges, universities, and technical colleges or schools that offer certification in a field approved by Employer; and WHEREAS, courses must be, in Employer s opinion, directly or reasonably related to employees present job or part of a degree program, or in line with a position that Employer believes employee can reasonably achieve; and WHEREAS, all full-time regular employees who have completed a minimum of one year of service are eligible to participate in the program; andWHEREAS, the undersigned Employee desires to participate in such program;NOW, THEREFORE, Employer and Employee agree as follows: A.Employee desires to enroll or is enrolled in the following school or technical college: , and is pursuing the following described degree or certification: B.Employee agrees that pursuing said degree or certification will not interfere with Employees job responsibilities.C. Reimbursement covers actual costs of tuition and registration fees only and is limited to a maximum of ______ credits per semester for approved courses, based on the following schedule: Grade Received Amount of Reimbursement A 100% B 75 P (pass/fail courses) 75 C 50 Lower than C 0D.If Employee is eligible for reimbursement from any other source (e.g., a government-sponsored program or scholarship) he/she may seek assistance under Employers educational assistance program but will be reimbursed only for the difference between the amount received from the other funding source and the actual course cost up to the maximum reimbursement allowable under this policy based on the grade received.E.To be eligible for reimbursement, Employee must (1) submit a tuition reimbursement form to his/her supervisor prior to the scheduled commencement of the course(s), (2) receive written approval from the supervisor and the human resources department in advance, (3) be actively employed by Employer at the time of course completion, and (4) receive a qualifying grade.F.On completion of the course, Employee must submit to the human resourcesdepartment an official transcript from the school, indicating the grade received, and a bursar's receipt or other proof of payment.G.
