Representing yourself without a lawyer is sometimes called appearing before the Court ‘pro se’, which come from Latin and means ‘for self’. If you want to represent yourself in this case, you should complete the form at the bottom o f the page and file this whole page with the Court. Be sure to include your complete mailing address and phone number(s) so the Court and the other parties can send you notice of court dates and copies of court papers. Each person must file a separate form, but you may mail them together in one envelope. You must send a copy of this appearance form to all the attorneys in the ca se and to all the other self-represented parties in the case.
