Children Traveling Abroad Information and Forms This package contains information about children traveling abroad with one parent, no parents or guardian. Documents Required The documents that may be required vary depending on the Country to be visited. Generally, the documents needed include the following: 1. Birth certificates showing the names of both parents. 2. Any legal documents pertaining to custody. 3. A parental consent letter i f the child is traveling with one parent , the consent letter authorizing travel must be signed and dated by the other parent, or if the child is traveling without either parent , the consent letter authorizing travel must be signed and dated by both parents. 4. A death certificate , if one of the parents is deceased. 5. If the child is traveling without both Parents, you may also want to consider providing a Power of Attorney (including health care decisions) to the person the child is traveling with.
