Form A 6/10/05 1 SUPREME COURT OF NEVADA CIVIL APPEALS PILOT PROGRAM INSTRUCTIONS FOR CIVIL LITIGANTS WITHOUT ATTORNEYS You are a party to an appeal in a civil matter pending in the Nevada Supreme Court. This appe al is part of a one-year pilot program established on June 10, 2005, by the enclosed order. The pilot program includes all civil appeals involving proper pe rson litigants. A proper person litigant is a party to an action who is not represented by an attorney in the Nevada Supreme Court. INSTRUCTIONS TO APPELLANT YOU ARE THE APPELLANT if you filed a notice of appeal from a judgment or order of the district court. IF YOU ARE THE APPELLANT, two fo rms are included with these instructions: (1) Civil Proper Person Appeal Statement Form (Required) (2) Proper Person Transcript Request Form (Optional) MANDATORY FILING : An appellant must file a Civil Proper Person Appeal Statement Form on or before the due date. The due date is 40 days after your appeal is docket ed in the Nevada Supreme Co urt. If you fail to file the Appeal Statement Form by the du e date, the Nevada Supreme Court may dismiss your appeal. OPTIONAL FILING : An appellant may file a Proper Person Transcript Request Form on or before the due date. The due date is 15 days after your appeal is docketed in th e Nevada Supreme Court. Although the filing of a Transcript Request Form is optional, if you fail to file this form, the Nevada Supreme Court will decide your appeal without reviewing transcripts of the trial court proceedings. HOW TO FILL OUT THE FORMS : All forms must be typed or legibly handwritten. Any form you file wi th the Nevada Supreme Court must be mailed or delivered to all other parties to this appeal or to the parties’ attorneys. You must also provide proof that you mailed or delivered the form as required. The court may return an y document that does not conform to these requirements. Form A 6/10/05 2 WHERE TO FILE THE FORMS : You may file your forms in person or by mail. You must file the or iginal and 2 copies with the Clerk of the Nevada Supreme Court. If you want the clerk to return a file-stamped copy of your form, you must submit the or iginal and 3 copies and include a self-addressed, stamped envelope. Documents cannot be faxed or e-mailed to the Clerk’s Office. To file your forms in person : Deliver them to th e Clerk’s Office at the Supreme Court of Nevada, 201 South Carson Street, Carson City, Nevada. The filing hours are Monday through Friday, 8:00 a.m. to 4:00 p.m. To file your forms by mail : Mail them to the Cl erk of the Supreme Court of Nevada, 201 S. Carson St., Carson City, NV 89701. Your forms must be postm arked by the due date. INSTRUCTIONS TO RESPONDENT YOU ARE THE RESPONDENT if you were a party to a district court action in which the opposing party ha s filed a notice of appeal. IF YOU ARE THE RESPONDE NT, you need not respond to any document filed by a proper person appellant unl ess the Nevada Supreme Court orders you to file a response. The court w ill not ordinarily grant appellant relief without an opportunity for you to file a response.
