I know of my own personal knowledge that ______ is the owner of the Oil and Gas Lease (the “Lease”) dated ______, between ______, as Lessor, and ______, as Lessee, recorded in Volume ______, page of the ______ Records of the county and state named above, covering the following lands in that county: (Description of Lands Covered by Lease) The Lease has a stated primary term of (Number of Years) from its date, and contains a provision that the Lease shall remain in force and effect as long thereafter as the Lessee produces oil or gas, or either of them, from the lands subject to the Lease. The owner of the Lease caused a well to be drilled on the Lease, which well wa s completed on or about (Month, Day, Year). Since that day, to the date of this Affidavit, that well has and continues to produce oil/gas in paying quantities. This Affidavit is made for the purpose of giving notice that the Lease has been ext ended beyond its stated primary term in the manner provided for in the Lease, and is deemed, by its terms, to continue to be in full force and effect.
