CONTRACT WITH INDEPENDENT CONTRACTOR THIS AGREEMENT entered into and effective the first day of **, 199** by and between **, Inc. ("**") and **, Inc. ("**"). NOW, THEREFORE, in consideration of the mutual covenants and promises contained herein and other good and valuable consideration, the receipt and sufficiency of which is here by acknowledged, the parties agree as follows: 1.
