INFORMANT/AGENT AGREEMENT 1.I release and acquit the ______________________________, its agents, and employees from any injury or liability which I may suffer or sustain in the future as a result of any investigation which I am directed to participate in. _____________2. I understand that no promises can be made as to the ultimate disposition of my pending case(s), whether criminal or civil in nature, other than the results of my assistance to the ____________________________, will be reported to the proper authorities involved in the jurisdiction of my case(s). _______________3. I understand that I am not to participate in any investigation of any criminal activity unless under the direct supervision of a sworn law enforcement official of the ___________________. _________4. I agree that I am to follow the instructions of the supervising agent/officer while assisting in any investigation. ___________5.I understand that I am instructed to not carry any weapon and/or firearm while I am participating in any investigation undertaken by the __________________. ________6. I understand that should I commit any crime during my term as a confidential informant, my participation as a confidential informant will be terminated immediately. ________7. I understand that in case of premature termination of my participation with the ________________, no recommendation or report of assistance will be given. ________8. I understand that while it is allowable for me to provide the opportunity for the commission of a crime, including reasonable steps to gain the confidence of suspect(s), it is not allowable for me to cause the commission of a crime by any act which might be construed as overbearing conduct on my part that is likely to cause a normally law-abiding person to commit a crime. ________ 11.I understand that I am not to use the _____________ or any law enforcement agency involved in any way with the ___________________ as any type of personal reference. ___________12.I understand that I am not privileged to break any laws during the course of my association with the _________________. __________13.
