Power of Attorney for Health Care - Statutory Form - This Statutory Power of Attorney for Health Care gives the person you designate as your agent/attorney in fact the power to make health care decisions for you. Your agent must act consistently with your desires as stated in this Power of Attorney. This document gives your agent the power to consent to your doctor not giving treatment or stopping treatment necessary to keep you alive. You have the right to make health care decisions for yourself as long as you can give informed consent. Revocation of Power of Attorney for Health Care – This Revocation of Statutory Power of Attorney for Health Care form is a revocation of the authority and power granted in Form NE- P014, which gives the person you designate as your agent/attorney in fact the power to make health care decisions for you. You may revoke a power of attorney for health care at any time while you are competent and in any manner (such as through this form) by which you communicate your intent to revoke. Declaration - Living Will - Statutory - This is a statutory form that is provided in the Revised Statutes of Nebraska. This document expresses your desires and wishes regarding whether to withhold or withdraw life-sustaining treatment that is not necessary for your comfort or to alleviate pain if you are unable to make that decision. Revocation of Declaration – This form is a revocation of NE-P024, Declaration, which provides for a person's wishes and desires regarding whether or not his/her life is prolonged by artificial means. Uniform Anatomical Gift Act Donation By An Individual Over 18 Years of Age – This Uniform Anatomical Gift Act Donation form pursuant to state statutes designates the specific body parts and organs an individual wishes to donate at the time of death. This form must be witnessed and the signature notarized. An anatomical gift may be made by a document of gift signed by the donor. Revocation of Anatomical Gift Donation - This Revocation of Anatomical Gift Donation form is a revocation of Form NE-P025 that designates the body parts and organs an individual wishes to donate at the time of death. A donor may amend or revoke an anatomical gift, not made by will, by a signed statement, by an oral statement made in the presence of two individuals, by any form of communication during a terminal illness or injury addressed to a physician or surgeon or by delivery of a signed statement to a specified donee to whom a document of gift had been delivered. An anatomical gift made by a will may be amended or revoked in the manner provided for amendment or revocation of wills or as set out above.
