Durable Power of Attorney for Health Care - This Pennsylvania Durable Power of Attorney designates a health care agent to express health care desires on your behalf when you are not capable of doing so yourself. It also includes living will declaration provisions with regard to specific life sustaining procedures and treatment. Statutory Living Will - Advance Directive for Healthcare - Statutory form - This Declaration form functions in the same manner as a living will and allows you to express your wishes and desires if it is determined that your death will occur whether or not life-sustaining procedures are utilized and where the application of life-sustaining procedures would serve only to artificially prolong the dying process. It is a declaration that such procedures be withheld or withdrawn, and that you be permitted to die naturally with only the administration of medication or the performance of any medical procedure deemed necessary to provide you with comfortable care. Revocation of Statutory Living Will - This is a revocation of the wishes and desires expressed in Form PA-P023, which is a form that allows you to express your wishes and desires if it is determined that your death will occur whether or not life-sustaining procedures are utilized and where the application of life-sustaining procedures would serve only to artificially prolong the dying process. This form acts as a revocation of a previously executed living will. This form complies with all applicable state statutory laws. Donation Pursuant to Uniform Anatomical Gift Act - This Uniform Anatomical Gift Act Donation form pursuant to state statutes designates the specific body parts and organs an individual wishes to donate at the time of death. An individual who is at least 18 years of age may make an anatomical gift by a signed document of gift. This form must be witnessed and the signature notarized. Uniform Anatomical Gift Act Donation Declaration - Any individual who is a minor and 16 years of age or older may effectuate a gift for any statutory purpose, provided parental or guardian consent is given. This form designates the body parts and organs which a person at least 16 years of age, with parental consent, wishes to donate at the time of death. Revocation of Anatomical Gift Donation - This Revocation of Anatomical Gift Donation form is a revocation of Form PA-P025 that designates the body parts and organs an individual wishes to donate at the time of death. A donor may amend or revoke an anatomical gift, not made by will, only by a signed statement, an oral statement made in the presence of two individuals, by any form of communication during a terminal illness or injury addressed to a physician or surgeon or by delivering a signed statement to a specified donee to whom a document of gift has been delivered.
