Power of Attorney for Health Care - Durable Power of Attorney for Healthcare – This document allows you to appoint an agent to make health care decisions for you and it gives your agent authority to consent, to refuse to consent, or to withdraw consent to any care, treatment, service, or procedure to maintain, diagnose, or treat a physical or mental condition. Revocation of Durable Power of Attorney for Health Care - This is a revocation of the authority granted in Form SD-P013 that allows you to appoint an agent to make health care decisions for you and to give your agent authority to consent, to refuse to consent, or to withdraw consent to any care, treatment, service, or procedure to maintain, diagnose, or treat a physical or mental condition. Statutory Living Will - This Statutory Living Will form allows you to express your wishes and desires if it is determined that your death will occur whether or not life-sustaining procedures are utilized and where the application of life-sustaining procedures would serve only to artificially prolong the dying process. It is a declaration that such procedures be withheld or withdrawn, and that you be permitted to die naturally with only the administration of medication or the performance of any medical procedure deemed necessary to provide you with comfortable care. Revocation of Statutory Living Will - This is a revocation of the wishes and desires expressed in Form SD-P023, which is a Statutory Living Will form that allows you to express your wishes and desires if it is determined that your death will occur whether or not life-sustaining procedures are utilized and where the application of life-sustaining procedures would serve only to artificially prolong the dying process. This form acts as a revocation of a previously executed living will. This form complies with all applicable state statutory laws. Anatomical Gift Act Donation – This Revised Uniform Anatomical Gift Act Donation form pursuant to state statutes designates the specific body parts and organs an individual wishes to donate at the time of death. This form must be witnessed and the signature notarized. Any individual of sound mind and fourteen years of age or more may give all or any part of his body for any purpose allowed by statute. If the individual is less than eighteen years of age, a parent or guardian must give his consent for the donation. An anatomical gift may be made by a will or by a document other than a will. Revocation of Anatomical Gift Donation - This form is a revocation of the anatomical gift(s) made in Form SD-P025, which designates the specific body parts and organs an individual wishes to donate at the time of death. If the document of gift has been delivered to a specified donee, the donor may amend or revoke the gift by executing and delivering to the donee a signed statement, making an oral statement in the presence of two persons and communicating the same to the donee, making a statement during a terminal illness or injury addressed to an attending physician and communicating the same to the donee, or keeping a signed card or document on his person or in his effects. If the document of gift has not been delivered to the donee, it may be revoked by the donor in the manner set out above or by destruction, cancellation, or mutilation of the document and all executed copies of the document.
