Statutory General Power of Attorney for Health Care – This form is provided in the Texas Health and Safety Code. It gives the person you name as your agent the authority to make any and all health care decisions for you in accordance with your wishes, including your religious and moral beliefs, when you are no longer capable of making them yourself. Revocation of Statutory Power of Attorney for Health Care - This form is a revocation of the authority granted in Form TX-P015 that gives the person you name as your agent the authority to make any and all health care decisions for you in accordance with your wishes, including your religious and moral beliefs, when you are no longer capable of making them yourself. Directive to Physicians and Family or Surrogates Statutory includes Living Will provisions - This form is provided in the Texas Health and Safety Code and is designed to help you communicate your wishes about medical treatment at some time in the future when you are unable to make your wishes known because of illness or injury. A competent adult may at any time execute a written directive. The directive must be signed by the principal in the presence of two witnesses (who must themselves sign the document). A declarant may include directions in a directive other than those provided in the statutes and may designate a person to make a treatment decision for the declarant in the event the declarant becomes incompetent or otherwise mentally or physically incapable of communication. Revocation of Directive to Physicians and Family or Surrogates l - This form is a revocation of Form TX-P021 that is designed to help you communicate your wishes about medical treatment at some time in the future when you are unable to make your wishes known because of illness or injury. You may revoke a directive at any time without regard to your mental state or competency by canceling, defacing, obliterating, burning, tearing, or otherwise destroying the directive or having someone do so for you, by signing and dating a written revocation such as this form that expresses your intent to revoke the directive or by orally stating your intent to revoke the directive. Uniform Anatomical Gift Act Donation - This Uniform Anatomical Gift Act Donation form pursuant to state statutes designates the specific body parts and organs an individual wishes to donate at the time of death. This form must be witnessed and the signature notarized. Any person of sound mind and eighteen (18) years of age or more may give all or any part of such individual's body for any purpose. An anatomical gift may be made by will or by a document other than a will. Revocation of Anatomical Gift Donation – This Revocation of Anatomical Gift Donation form is a revocation of Form TX-P025 that designates the body parts and organs an individual wishes to donate at the time of death. If the will, card or other document of gift has been delivered to a specified donee, the donor may amend or revoke the gift by executing and delivering to the donee a signed statement, making an oral statement in the presence of two persons and communicating the same to the donee, making a statement during a terminal illness or injury addressed to an attending physician and communicating the same to the donee or by keeping a signed card or document on his person or in his effects. Any document of gift which has not been delivered to the donee may be revoked by the donor in the manner set out above or by destruction, cancellation, or mutilation of the document and all executed copies thereof.
