Power of Attorney and Health Care Revocation for Connecticut – This is a revocation of the appointment made in Form CT-P020. Statutory Health Care Directive Living Will – This is a statutory form that is provided for in the General Statutes of Connecticut that allows you to express your wishes and desires regarding whether or not your life is prolonged by artificial means. Any person eighteen years of age or older may execute a document which contains directions as to specific life support systems which that person chooses to have administered. The document must be signed and dated by the maker with at least two witnesses. Revocation of Statutory Health Care Directi ve - This form is a revocation of Form CT-P021, which allows you to express your wishes and desires regarding whether or not your life is prolonged by artificial means. You may revoke such wishes and desires in any manner, including this form, at any time without regard to your mental or physical condition. Donation Pursuant to Uniform Anatomical Gift Act - This Uniform Anatomical Gift A ct Donation form pursuant to state statutes designates the specific body parts and organs an individual wishes to donate at the time of death. An individual who is at least 18 years of age may m ake an anatomical gift by a signed document of gift. This form must be witnessed and the signature notarized. Revocation of Anatomical Gift Donation - This Revocation of Anatomical Gift Donation f orm is a revocation of Form CT-P025 that designates the body parts and organs an individual wishes to donate at the time of death. A donor may amend or revoke an anatomical gift, not made by will, only by a signed statement, an oral statement made in the presence of two individuals, by an y form of communication during a terminal illness or injury addressed to a physician or surgeon or by delivering a signed statement to a specified donee to whom a document of gift has been delivered.
