EXHIBIT "A" TO SUBPOENA DUCES TECUM 1. A complete copy of all claim files, including, but not limited to, the claim file maintained by the claims adjuster to whom this claim was assigned, which touch or concern the automobile accident made the basis of the Plaintiffs' complaint; compliance with this request shall include the claim files maintained by persons other than the claims adjuster to whom this claim was assigned, including, but not limited to, successor adjusters, immediate supervisors, district or regional claims managers, or other persons affiliated with the insurance company; 2. A complete copy of all investigative files, including, but not limited to, the investigative file maintained by the adjuster to whom this claim was assigned, which touch or concern the automobile accident made the basis of the Plaintiffs' complaint; compliance with this request shall include the investigative files maintained by persons other than the adjuster to whom this claim was assigned, including, but not limited to, successor adjusters, immediate supervisors, district or regional claims managers, or other persons affiliated with the company; 3. A complete copy of all underwriting file(s) with respect to all policies of insurance covering the Defendant or Defendant's immediate family members in the past ten years; 4. An attested copy of all policies of insurance in force and effect as of             , 20       ; 5. Each and every declarations page associated with and accompanying the policies referenced in number 4 above; 6. Each and every claims file compiled by your company, regardless of location, preparer(s), or otherwise, which relate to any and all claims for coverage, indemnity or defense by or on behalf of the Defendant during the preceding ten years; 7. A complete copy of each and every claim file or investigative file prepared by entities or persons on behalf of your company which relate to the accident made the basis of plaintiffs' complaint and which is in the possession, custody or control of your company; 8. A complete copy of all written or recorded statements taken by or on behalf of your company which relate to the claims made the basis of plaintiffs' complaint or which relate in any way to the accident which occurred on             , 20       ; 9.
