Whereas, by Decree ( or Judgment ) of the ________________________________ (Name of Court) of _______________________________________________ (Name of County and State) , located at ________________________________________________ ________________________________________________________________________ ___________________________________ (street address, city, county, state, zip code) , the marriage of FH and FW was terminated; and Whereas, pursuant to said Decree (or Judgment ), FH was ordered to pay spousal support to FW in the amount of $___________________ ($Amount) per month until the ___ day of __________________________________ (month and year) ; and Whereas, due to a change in the financial circumstances of FW, FH has agreed to continue to pay to FW the amount of $___________________ ($Amount) on the ______ day of each month until the ______ day of ______________________________ (month and year) ; Now, therefore, for and in consideration of the mutual covenants contained in this agreement, and other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, the parties agree as follows: FH shall continue to pay to FW spousal support in the amount of $___________________ ($Amount) on or before the _____ day of each month, and continuing until the ____ day of _________________________________ (month and year) .
