That Petitioner, __________________ , is the person named as minor in the above- entitled matter. 2. That Petitioner turned twenty-one (21) years of age on _____________, 20____. 3. That Petitioner has made a full and complete settlement with the guardian, and has received from the guardian all of the money and property in the hands of the guardian belonging to petitioner. 5. That Petitioner waived the issuance of any notice of hearing on the final account of the guardian, and submitted himself to the jurisdiction of this court and asked that the final accounting of the guardian, be waived. IT IS, THEREFORE, ORDERED, ADJUDGED AND DECREED as follows: (a) That guardian be discharged; (b) That the liability of surety on the guardian's bond be terminated; and (c) That the guardian not be required to file any further account in the matter. (d) That upon taking such actions, the said Guardianship be terminated.
