This form is a guide to help you in meeting various requirements, such as voting, identification card, driver’s license and vehicle registration requirements. VOTING REGISTRATION REQUIREMENTS - In order to register to vote in the state of Hawaii, you must meet the following requirements:  You must be a citizen of the U.S.  You must be a legal resident of Hawaii.  You must be at least 18 years of age. You may pre-register at age 16, but you must be 18 years old by election day to vote.  You must complete a voter registration form by one of the following methods: o By visiting the Office of the City or County Clerk where you reside to complete an Affidavit on Application for Voter Registration. o By simultaneously applying to register to vote when you apply for a driver’s license at the Department of Motor Vehicle. The State of Hawaii Application for Motor Vehicle Driver’s License also contains a Motor Voter Affidavit on Application for Voter Registration. o By obtaining and completing an Affidavit on Application for Voter Registration brochure at your satellite city hall, public library, U.S. Post Office, state services agency, University of Hawaii System or the Office of Elections’ website at http://hawaii.gov/elections and mailing it to the Office of the City or County Clerk where you reside. Any person who knowingly furnishes false information on the Affidavit on Application for Voter Registration may be guilty of a Class C Felony, punishable by up to 5 years imprisonment and/or $10,000 fine. IDENTIFICATION CARD REQUIREMENTS - In order to obtain an identification card in Hawaii, you must generally meet the following requirements:  You must come in person to a state ID office, with the proper documents and fee. The fee is $15.00 in cash except for those ages 65 and older, for which the fee is $10.00 in cash.  You must provide legal, certified documents to support any changes in your status or name due to marriage, divorce, annulment, adoption, or citizenship.  You must present an original Social Security card.  You must present one of the following additional documents: o A certified birth certificate issued by the state repository in the state where you were born; or o A certificate of citizenship or naturalization; or o A document for a child born abroad of American parents; or o A valid United States Passport if issued in your legal name.  If you are under the age of 14, you must have a parent or guardian sign your application form and show proof of guardianship, if applicable. DRIVER’S LICENSE REQUIREMENTS - The state of Hawaii does not have a statewide Department of Driver Licensing. Driver licensing is managed by each county government. You are not required to be a U.S. citizen or a Hawaii resident to obtain a Hawaii driver’s license. Generally, if you hold a valid out-of-state driver’s license, you must meet the following requirements to obtain a Hawaii driver’s license:  You must complete all of the requirements for an instruction permit. The examiner, however, may waive the road test for persons 18 years of age and older. You will retain your valid out-of-state license and not receive a permit until you successfully complete any required practical test. You must successfully complete an eye exam and a knowledge test.  You must surrender your out-of-state driver’s license prior to being issued a Hawaii driver’s license.  You must present your social security number and proof of your name and age. You may also need to present a valid driver’s license, birth certificate, military ID, along with other supporting documents. V EHICLE TITLE AND REGISTRATION REQUIREMENTS - Each county in the state of Hawaii has its own vehicle title and registration requirements. Generally, in order to obtain a title and registration for your vehicle in the state of Hawaii, you must meet the following requirements:  You must complete an application for registration form, which must be signed by the owner(s) of your vehicle.  You must submit your last issued out-of-state certificate of registration and certificate of title if you are listed as the lienholder.  You must submit a bill of lading or shipping receipt showing the date your vehicle arrived in Hawaii.  You must submit a Hawaii certificate of safety inspection.  You may also be required to submit certain other forms for particular vehicle year models.
