Any person who knowingly furnishes false information on the Affidavit on Application for Voter Registration may be guilty of a Class C Felony, punishable by up to 5 years imprisonment and/or $10,000 fine. IDENTIFICATION CARD REQUIREMENTS - In order to obtain an identification card in Hawaii, you must generally meet the following requirements:  You must come in person to a state ID office, with the proper documents and fee. The fee is $15.00 in cash except for those ages 65 and older, for which the fee is $10.00 in cash.  You must provide legal, certified documents to support any changes in your status or name due to marriage, divorce, annulment, adoption, or citizenship.  You must present an original Social Security card.  You must present one of the following additional documents: o A certified birth certificate issued by the state repository in the state where you were born; or o A certificate of citizenship or naturalization; or o A document for a child born abroad of American parents; or o A valid United States Passport if issued in your legal name.  If you are under the age of 14, you must have a parent or guardian sign your application form and show proof of guardianship, if applicable. DRIVER’S LICENSE REQUIREMENTS - The state of Hawaii does not have a statewide Department of Driver Licensing. Driver licensing is managed by each county government.
