This form is a guide to help you in meeting various requirements, such as residency, voting, identification card, driver’s license and vehicle registration requirements. RESIDENCY REQUIREMENTS - To be domiciled in Oregon, your primary home must be in Oregon and you must intend to remain in Oregon or, if absent, to return. Other factors indicating residency in Oregon include the following:  Remaining in Oregon for a consecutive period of six months or more each year.  Enrolling children in an Oregon public school without payment of nonresident tuition fees.  Paying Oregon resident tuition fees at an educational institution maintained by public funds.  Making a declaration to be a resident of Oregon for the purpose of obtaining, at resident rates, a state license (other than an Oregon Driver’s License).  Listing Oregon as home of record in military records.  Paying resident income taxes to the state of Oregon. VOTING REGISTRATION REQUIREMENTS - In order to vote in the state of Oregon, you must meet the following requirements:  You must be a U.S. citizen.  You must be a resident of Oregon.  You must be 18 years old by Election Day.  You must complete a voter registration form. If you are registering in Oregon for the first time, your completed voter registration form must be postmarked by the 21st day before an election in order to vote in that election. You may obtain a voter registration form as follows: o Online at http://www.sos.state.or.us/elections/votreg/sel500.pdf . You may complete the form online, print and sign it, or print the form and complete it using black ink. You must mail it to your County Elections Office or drop it off in person. o From your County Elections Office, Secretary of State Office, other state agencies, such as the Division of Motor Vehicles or at a voter registration drive.  If you register by mail and it is your first time to register in Oregon, you must include a copy of one of the following valid forms of identification showing your current name and address: o Current photo identification (such as driver’s license) o Paycheck stub o Utility Bill o Bank statement o Government document. IDENTIFICATION CARD REQUIREMENTS - You may obtain an Oregon non-driver identification card for use for identification purposes regardless of your age. In order to obtain an identification card in Oregon, you must meet the following requirements:  You must be a resident of or be domiciled in Oregon.  You must not already have a valid Oregon ID Card or a current valid Driver License or Instruction Permit from Oregon or any other jurisdiction.  You must apply for an original ID card in person at a Driver and Motor Vehicles Division Office by: o Completing the Driver License/Identification Card form; o Presenting proof of age, identity and residence as required below for driver’s licenses; o Paying the original application fee; and o Surrendering any valid driver’s license or permit issued by Oregon or any other jurisdiction. DRIVER’S LICENSE REQUIREMENTS - Once you have become a resident of Oregon, you are required by Oregon law to obtain an Oregon Driver’s License or Instruction Permit. Failure to obtain an Oregon Driver’s License or Instruction Permit once residency has been established is a Class B traffic violation. In order to obtain a driver’s license in Oregon, you must meet the following requirements:  You must be at least 18 years of age.  You must be a resident of or domiciled in Oregon.  You must not have suspended or revoked driving privileges in Oregon or any other state.  You must provide proof of age and identity in the form of two primary documents or one primary document that includes your name and date of birth, plus two secondary documents. Primary documents include the following: o A U.S., Canadian or U.S. Territorial government-issued birth certificate o A U.S. Consular Report of Birth Abroad (FS-240) o A U.S. Government issued Certification of Birth (DS-1350 or FS- 545) o U.S. military documents, including:  Military or Armed Forces ID Card;  Common Access Card;  U.S. Uniform Services ID and Privileges Card; and (DD1173 and DD1173-1)  Request for Verification of Birth (DD372). o Passport (acceptable up to five years after expiration) o U.S. immigration documents including:  U.S. Citizen ID Card (I-179 and I-197);  Temporary Resident ID Card (I-688);  Employment Authorization Document (I-688A, I-688B, and I- 766);  Certificate of Citizenship (N560 and N561);  Resident Alien Card or Permanent Resident Card (I-551); or  Certificate of Naturalization (N550, N570, and N578). o Out-of-state, U.S. Territorial government, District of Columbia or Canadian Driver’s License, Instruction Permit or Identification Card with your photograph (acceptable up to one year after expiration) o Oregon Driver’s License, Instruction Permit or Identification Card with your photo (acceptable up to five years after expiration) o A photograph on file with Oregon Driver and Motor Vehicles Division o Non-Immigrant Visa issued by the U.S. Department of State o U.S. Department of State Driver’s License or Non-Driver ID Card o A Consulate ID Card approved by Driver and Motor Vehicles Division o Oregon Concealed Weapons Permit/Concealed Handgun License o U.S. Social Security Card o A Confederated Tribes of Oregon Tribal ID Card approved by Driver and Motor Vehicles Division o Letter verifying identity provided by an Oregon Correction agency, approved by Driver and Motor Vehicles Division o Letter verifying identity provided by U.S. Pretrial Services, approved by Driver and Motor Vehicles Division o Letter verifying identity provided by U.S. District Court Probation Office, approved by Driver and Motor Vehicles Division o Letter verifying identity provided by Oregon Youth Authority, approved by Driver and Motor Vehicles Division o Oregon Department of Corrections Release ID card issued after April 30, 2005 (must state "Verified Name.”) Secondary documents include the following: o An Oregon Student Body or Oregon Job Corps Identification Card, issued for the current school year or not more than one year old o U.S. Military Discharge Papers (DD214) o Permit to Reenter the U.S. (I-327) o Border Crossing Cards (DSP-150, I-185, I-186, and I-586) o Refugee Travel Document (I-571) o W-2 and 1099 tax forms for the current tax year o Medicare cards o Social Security transmittal document (stub) o Social Security benefits letter dated within the last year o Court documents issued by a court in the U.S. which show you as a party to the judicial proceeding, not as third party "named" in the document, and the document must contain court signatures and seals. Acceptable documents are:  Gender and/or name change  Adoption, guardianship, custody or child support  Divorce decree dissolution of Marriage o U.S. Government issued Marriage License/Certificate (must be signed by a government official or indicate that it has been filed) o Veterans’ Universal Access Identification Card o Birth certificate, driver’s license, military ID card, passport or voter card issued by a foreign government.  If you do not have an Oregon Driver’s License, Permit or ID Card or you have a change of address and your current address is not shown on any of the documents you are presenting as proof of identity, you must submit proof of residency in the form of one of the following documents: o Mortgage documents issued within the current or immediately preceding year o Statement from your parent, step-parent or guardian who resides at the same address (parent, step-parent or guardian must accompany you and present identification) o Statement from your spouse who resides at the same address (spouse must accompany you and present identification) o A rental or lease agreement on an official pre-printed application form dated within one year of the application which contains:  The name of the tenant;  The address of the rental unit; and  The name and signature of the rental owner or manager. o A utility hook up or work order dated within 60 days of the application o Payment booklet o Mail dated within 60 days of the application from the following sources:  Credit card companies  U.S. Treasury  Social Security Administration (including benefits letter)  Oregon State government agencies (including city and county agencies)  Utility companies  Financial institutions (including checking, savings, or investment account statements)  Insurance companies  State and Federal Revenue Departments  Originators of out-of-state clearance letters o Oregon vehicle title or registration documents o Oregon voter registration card o Selective Service card o Medical or health card o Educational institution transcript forms for the current school year o Unexpired professional license issued by an agency in the U.S. o Form DS2019, Certificate of Eligibility for Exchange Visitor (J-1) status  You must complete a Driver License and Identification Card Application.  You must provide your social security number.  You must pass the driver license (Non-Commercial Class C) knowledge test at a regular or full service Driver and Motor Vehicles Division office.  If you have a valid driver’s license issued by a U.S. State, you may not be required to take the drive test. Otherwise, you must take the behind-the- wheel drive test conducted by the Driver and Motor Vehicles Division in a Non-Commercial Class C vehicle and provide proof of insurance.  You must maintain the following minimum insurance coverage amounts: o Bodily injury and property damage liability coverage of $25,000 per person, $50,000 per crash for bodily injury to others and $10,000 per crash for damage to the property of others o Personal injury protection coverage (for reasonable and necessary medical, dental and other expenses one year after a crash) of $15,000 per person o Uninsured motorist coverage of $25,000 per person and $50,000 per crash for bodily injury. You may show proof of insurance in the form of one of the following: o A valid insurance card. o A current motor vehicle liability insurance policy or binder. o A letter signed by an authorized agent or insurance company official, on agent or insurance company letterhead. o A Driver and Motor Vehicles Division Certificate of Self-Insurance naming the owner of the vehicle. o A displayed Oregon auto dealer license plate unless the dealership does not sell motorized vehicles and has completed a "DMV Certificate of Exemption from Vehicle Liability Insurance".  You must pass a vision screening.  You must surrender any valid driver’s license or permit issued by another jurisdiction (such as another state).  You must pay the appropriate testing and issuance fees. In order to obtain a provisional driver’s license in Oregon (if you are 16 or 17 years old), you must meet the following requirements:  You must provide proof of residency or domicile in Oregon.  You must be at least 16 years of age (16 or 17 years old).  You must provide your social security number.  You must provide proof of identity and residence address.  You must have had an instruction permit from Oregon, another state or the District of Columbia for at least six months. (The instruction permit must have been valid for at least six months in the issuing state or District of Columbia).  If you are applying for an Oregon driver’s license for the first time, you must provide proof of school enrollment, completion or exemption.  You must complete an ODOT-approved traffic safety education course (offered through a ODOT-Approved (commercial) Driver Education Provider or through a school, school district or Educational Service District) and present a completion certificate with the ODOT approved certification seal, or complete an additional 50 hours of supervised driving experience certified by your parent or legal guardian. (Your supervising driver must be at least 21 years old and must have had a valid driver’s license for at least three years).  You must have your parent or legal guardian certify that you have at least 50 hours of supervised driving practice.  You must complete a Driver License/Identification Card form and obtain your parent or legal guardian’s signature on it (unless you are married or you are an emancipated minor), and bring the completed, signed form to a Driver and Motor Vehicles Division office.  You must pass a Safe Driving Practices Test.  You must pass a vision screening test.  You must take the behind-the-wheel drive test conducted by the Driver and Motor Vehicles Division in a non-commercial Class C vehicle and provide proof of vehicle insurance.  You must pay the appropriate testing and issuance fees.  You must surrender any valid driver’s license or permit issued by another jurisdiction (such as another state).  You must not have suspended or revoked driving privileges in Oregon or any other state. For the first six months after you get your provisional driver’s license, you can not drive with a passenger under age 20 who is not a member of your immediate family. For the second six months after you get your provisional driver’s license, you can not drive with more than three passengers who are under age 20 who are not members of your immediate family. For the first year after issuance of your license, you can not drive between midnight and 5am unless you are:  Driving between home and work;  Driving between home and a school event for which there is no other transportation available;  Driving for employment purposes; or  Accompanied by a licensed driver who is at least 25 years old. The above restrictions only apply until you are 18 years of age or you have had your provisional license for one year, whichever comes first. VEHICLE TITLE AND REGISTRATION REQUIREMENTS - Once you have become a resident of Oregon, you are required by Oregon law to obtain Oregon vehicle title and registration. Failure to register a vehicle once residency has been established is a Class D traffic violation. In order to register and title your vehicle in Oregon, you must meet the following requirements:  You must be a resident of or be domiciled in Oregon.  You must certify to one of the following statements: o You are domiciled in Oregon. o Your vehicle is required to be registered because you have established Oregon residency. o Your vehicle meets all of the following qualifications for Oregon registration:  Your vehicle is usually left in Oregon when you are away from Oregon.  Your vehicle is used primarily for personal transportation in Oregon.  Your vehicle is a private passenger vehicle or a vehicle having a loaded weight of less than 10,000 pounds.  Your vehicle is not a motor home or a camper.  If you live in the Portland Area or the Rogue Valley (Medford), you must obtain an emissions test at the Department of Environmental Quality (DEQ). You may also be able to get your Vehicle Identification Number inspection while at DEQ, if applicable.  You must complete an Application for Title and Registration.  If the title is not in your name, you must provide releases of interest from previous owners on: o Vehicle Title o Bill of Sale o Lien Release.  You must choose your license plates.  You must bring your vehicle and title to a Regular or Full Service Driver and Motor Vehicles Division Office to get a VIN inspection, if you did not get your VIN inspection at DEQ.  You must pay the following fees: o Title transaction fees o Registration fees o Plate fees o VIN inspection fee. If you took out a loan to purchase your vehicle or are leasing it and the lienholder possesses the title of your vehicle, you will need to meet the following requirements to title and register your vehicle in Oregon:  You must complete a Security Interest Holder Letter and send it to your bank or finance company. Your security interest holder must add your title to the documents below and mail them back to the Driver and Motor Vehicles Division.  If you live in the Portland Area or the Rogue Valley (Medford), you must get an emissions test at DEQ. You may also be able to get your VIN while at DEQ.  You must obtain and complete an Application for Title and Registration.  You must bring your vehicle to a regular or full service Driver and Motor Vehicles Division office immediately upon establishing Oregon residency to obtain a VIN inspection (if you did not get a VIN inspection at DEQ) and to turn in your title documents.  You must choose your license plates.  You must pay the appropriate fees.
