Agency Telephone: _____________________________________ Agency Address: _____________________________________ _____________________________________ Or Appointing Guardian ad litem (ORAPGL) - Page 1 of 2 WPF JU 03.0300 (06/2014) - RCW 13.34.100, .105 2.4 The appointed guardian ad litem has completed the required guardian ad litem training, and has no criminal history for 10 years prior to this appointment as per RCW 13.34.100(3). 2.5 The guardian ad litem shall receive all notice and documents contemplated for a party in all proceedings in this manner including full discovery of all documents filed with the court in the above proceeding in accordance with the Civil Rules Procedure of the Superior Court. 2.6 The guardian ad litem shall have access to the minor child and information about the child. All parties and their attorneys shall cooperate fully in providing said access to the guardian ad litem and in providing all requested information. 2.7 The guardian ad litem shall inform the child, if the child is twelve years old or older, of his or her right to request an attorney and to ask the child whether he or she wishes to have an attorney, pursuant to RCW 13.34.100(7). The guardian ad litem shall report to the court that the child was notified of this right and indicate the child's position regarding appointment of an attorney. The guardian ad litem shall report to the court his or her independent recommendation as to whether appointment of an attorney is in the best interest of the child. 2.8 In the case of an Indian Child as defined in RCW 13.38.040, the guardian ad litem shall know, understand, and advocate the best interests of the Indian child. 2.9 Except for information or records specific in RCW 13.50.100(7), the guardian ad litem shall have access to all information available to the state or agency on the case. Upon presentation of the order of appointment by the guardian ad litem, any agency, hospital, school organization, division or department of the state, doctor, nurse, or other health care provider, psychologist, psychiatrist, police department, or mental health clinic shall permit the guardian ad litem to inspect and copy any records relating to the child(ren) involved in the case, without the consent of the parent or guardian of the child(ren), or of the child(ren) if the child(ren) is/are under the age thirteen years, unless access is otherwise specifically prohibited by law. 2.10 The guardian ad litem shall not release case information except in accordance with the provisions of RCW 13.50.100 2.11 Other: .
