Plaintiff alleges that the mother is a fit and proper person for the custody of the minor child and that the child's best interest would be served by granting Plaintiff custody subject only to supervised visitation rights of the Defendant. WHEREFORE, PREMISES CONSIDERED, Plaintiff prays that upon a final hearing in this matter, the court will award her the following permanent relief: 1. That this court enter a judgment of filiation that the Defendant is the father of the child. 2. That the child shall inherit from the Defendant as could the natural children of the father under the statutes of decent and distribution in this state. 3. A reasonable amount from the Defendant as child support for support and maintenance of the minor child for the remainder of the child's minority. 4. That the Defendant be ordered to pay back child support for the space of       year(s) prior to the filing of this Complaint. 5. That the Defendant be ordered to provide health insurance and pay       (       ) of any and all medical expenses not covered under said insurance for the minor child. 6.
