STATE OF MINNESOTA DISTRICT COURT COUNTY OF       JUDICIAL DISTRICT FAMILY COURT DIVISION In Re the Marriage of: Court File No. , Petitioner, STIPULATION AND ORDER FOR AMENDED JUDGMENT AND DECREE AND , Respondent, WHEREAS, the marriage of the parties hereto was dissolved by the entry of a Findings of Fact, Conclusions of Law, Order for Judgment and Judgment and Decree entered on _____________ , 20 ___ and WHEREAS, paragraph 3 of the Conclusions of Law of the above-referenced Judgment and Decree contains the Court's Order with respect to continued medical and dental insurance for Respondent through Petitioner's employment; WHEREAS, the parties desire to amend the above-referenced paragraph 3 to more particularly clarify the entitlements to Respondent with respect to medical and dental coverage; and NOW, THEREFORE, IT IS HEREBY STIPULATED AND AGREED: 1. That paragraph 3 of the Conclusions of Law of the Amended Findings of Fact, Conclusions of Law, Order for Judgment and Judgment and Decree entered on _____________ , 20 ___ , shall be vacated and amended to read as follows: 6. Health Insurance . That the Petitioner maintains health and dental insurance plans through her employer, Northwest Orient, which covers the Respondent. The parties have been informed by Northwest Orient that Respondent can continue to have medical coverage until either of the parties remarry. That Petitioner shall maintain Respondent on said medical plan until Petitioner or Respondent remarries. In addition, Respondent shall have any continuation or conversion rights for coverage under said medical and dental plans as available under state and - 1 - federal law. Respondent shall be responsible for any premium or other costs necessary for him to have coverage under said plans. Each party shall keep the other party advised of a current mailing address so long as any such insurance is maintained. 2. That the Findings of Fact, Conclusions of Law, Order for Judgment and Judgment and Decree entered herein on February 13, 1989, shall in all other respects remain in full force and effect. 3. Copies of this Order shall be sent by U.S. Mail to each party, which shall be sufficient and proper service. 4. That the above amendment to paragraph 3 of the Conclusions of Law shall be effective nunc pro tunc as of the entry of the original Judgment and Decree.
