- 1 – Copyright 2004 USLegalforms.com PARENT-CHILD ALCOHOL AND DRUG AWARENESS CONTRACT This Agreement (“Agreement”) is entered into as of this day of 20 , by and between and (hereinafter referred to as PARENT(s)) and , (hereinafter referred to as CHILD) who reside at in the state of for purposes of establishing boundaries regarding the use of alcohol and drugs. WHEREAS, CHILD acknowledges that underage drinking is illegal and can influence said C HILD in a negative manner. WHEREAS, CHILD acknowledges that illegal drugs of any kind are harmful and will result in serious repercussions for said CHILD. Now, THEREFORE, in consideration of the material promises set forth below the parties hereby agree as follows: 1. This Agreement is comprised of this document and shall incorporate any amendments, provided they are properly written and executed as provided herein. The terms of this Agreement shall control over any conflicting terms contained in ancillary documents. 2. This Agreement shall become effective when signed by both parties and shall conti nue in full force and effect until 20 . 3. At no time shall alcohol be brought into CHILD’S room or this residence at the aforementioned address. 4. At no time shall CHILD engage in the use of illegal or illicit drugs of any kind including the use of any mind-altering substances or prescription drugs. 5. At no time shall paraphernalia or drug related materials be in CHILD’S possession or personal belongings. 6. CHILD shall submit to a random drug test should PARENT (S) suspect the use of any such substanc es without having to provide CHILD with any warning that such test will be conducted. 7. CHILD shall abide by the terms of the contract and shall not be granted any privil eges until full compliance is made with the terms and conditions as stated. THIS IS NOT NEGOTIABLE. 8. Privileges afforded to CHILD are at the discretion of PARENT (S) and shall be as follows: 9. This Agreement may be renegotiated at the following date 20 . Revision or renegotiation is based upon CHILD’S progress. CHILD will earn more or fewer privileges based on behavior demonstrated throughout the duration of time that this Agreement is in effect. 10. Should CHILD choose to break the terms of this Agreement then the following sanctions will apply: a. First time infraction- b. Second time infraction- c. Third time infraction- AN INFRACTION MEANS BREAKING THE TERMS AND CONDITIONS OF THIS AGREEMENT IN ANY MANNER, FORM OR FASHION, INCLUDING BUT NOT LIMITED TO, BEING IN THE HOUSE AFTER HAVING CONSUMED ALCOHOL, DRINKING AND DRIVING, EXPERIMENTING WITH ILLEGAL SUBSTANCES ETC. - 2 – Copyright 2004 USLegalforms.com 11. PARENT (S) agrees to note any instances of good behavior and grant privileges accordingly. 12. PARENT (S) agrees that he/she/they are acting only in the best interest of said CHILD and are looking out for the welfare of said CHILD by prohibiting the use of drugs and alcohol.
