Fill and Sign the Permission Photograph Form

MODEL PERMISSION TO PHOTOGRAPH In consideration of my engagement as a model, and for other good and valuable consideration herein acknowledged as received, I,       hereby grant       permission to photograph me. I further give my irrevocable consent to said photographer or agency to publish, republish, copyright, sell, reuse, display all and any image or likeness of me in any medium and in any forum for all purposes throughout the world. This permission covers any and all uses and purposes, and I understand and acknowledge that the images may be altered or modified in any manner, form or fashion. In addition, I hereby waive any right to inspect and approve any negative, impression, slide, or finished product or copy that may have been used in connection with an image that said photographer may have taken of me. Without limitations, I further release and discharge and hold harmless the aforementioned photographer and/or agency from any claims of invasion of privacy or defamation of character. Photographer and any assignees or licensees is released from any claims for remuneration associated with any type of damage, whether foreseen or unforeseen, associated with the use of the image(s).       Name of Model       Date ___________________ Signature
