Fill and Sign the Permission to Use Students Work Form

PERMISSION TO USE STUDENT’S WORK I,       hereby grant       permission to use for educational or promotional purposes, a royalty-free, non-exclusive, perpetual right and license to exercise any and all rights under copyright in my original student writing or disquisition the following works                   in whole or in part, for the duration of the copyright. This permission does not accompany any right to publish or distribute my work commercially. All right, title, and interest in the work, including any copyright as identified in Title 17 of the United State Code,  107 shall remain with me. I acknowledge that this permission entitles the aforementioned entity to use my student work, in whole or in part, without my prior notification. Further, I agree to save harmless and release from liability and any cause of action, including but not limited to any claims alleging an infringement of intellectual property rights or rights of any third party, the aforementioned entity as well as its employees, officers, directors, agents, assigns, students, or any persons acting under their direction, permission or supervision.
